Living with a cancer diagnosis presents many physical, emotional, spiritual, social, and practical needs. This is particularly challenging in a rural community such as ours, where supportive care resources are at a premium. Blueprints of Hope is the only community-based oncology nurse navigation and supportive care program in Southwest Colorado.
Blueprints of Hope was founded in 2012 by an oncology nurse who was faced with the difficulty of meeting the needs of her patients during the normal course of her day in an oncology nursing setting. She worked long hours (as most cancer nurses do) with innumerable patients to try to find the resources to meet their needs in the clinical cancer setting. She decided to hang up her “day” job and begin a program in rural Southwest Colorado to try to discover the resources and services to make lives better individuals facing a cancer diagnosis in her cancer circle. Her vision was to work toward making lives better – in the community setting, away from medical care – and get patients and survivors back to living their best lives, no matter which phase of cancer they were experiencing.
Since 2012, Blueprints of Hope has had the opportunity to consolidate cancer support communities for cancer survivors into one community calendar, provide integral support for those who have had mastectomies, those who are facing lymphedema and other rehabilitation issues, those who need help finding ostomy supplies, and who are looking for psychosocial supportive care programs and services, and to educate and attend physician and oncology visits with clients who are facing the cancer journey solo. One of the core standards of Blueprints of Hope's programming is to provide cancer nurse navigation (guidance) to any individual who is alone, and who hasn’t any support system when facing a cancer diagnosis.
Blueprints of Hope has also partnered with the Colorado Live by Living Walking and Hiking program to provide a shared exercise support system for any individual who is facing cancer.
Whether they are empowering clients to secure transportation so they can make it to all of their scheduled treatments, to locate financial resources, childcare, legal services, find a safe home, provide advocacy and a second set of ears, help individuals get back to work, educate about therapy and their side effects – Blueprints has been empowering our clients to locate these services and make informed decisions about their care.
