Building Healthy Communities
Organized by: Diana Tran
EVENT DATE Nov 08, 2014
Dear Friends and Family,
As you may already know, I have been planning a health fair to take place on November 8th, 2014 in San Bernardino, CA.
San Bernardino County is the largest county in the United States, with a population of two million living in diverse geographic and civic environments. The county ranks 50th out of 56 California counties in health factors. The health problems facing this region make it imperative that we give priority to the health needs of our community. It is for this reason that it is my commitment to connect residents with the health and social services that they critically need. This health fair is my first step to deliver accessible services and to expand outreach programs with the aim of encouraging the underserved population to get regular routine care.
With the help of community partners, my classmates and I will be providing basic health screenings, flu immunizations, blood pressure and glucose checks, eye exams, free prescription glasses, foot exams, hearing exams, and exercise classes as well as a variety of community resources and programs. The health fair will be open to both adults and children. We anticipate attendance of approximately 500 people. This event is free to the public. But, we need your support to make that happen.
My goal is to raise $1200 by November 1st to cover the costs of the venue, rentals, medical supplies, health goodie bags, and food/refreshments for volunteers. No donation is too small and all donations are tax deductible (Tax ID#: 95-6006142). Please let me know if you need an official sponsorship form.
For more information on sponsorships or regarding the event, please contact me anytime at firstname.lastname@example.org. I truly appreciate your consideration of this request and thank you in advance for your support! I know that with friends, family, and colleagues like you, I will be able to make a difference in lives, one patient at a time.
