Since 1965, York County Community Action Corporation has been delivering a range of social service, health, and educational programs to York County individuals and families living in poverty. Every year, YCCAC staff interacts with 22,000+ low-income residents. Our major programs include Head Start/Early Head Start, WIC, Energy Assistance, Transportation, Housing Services, Community Outreach, and our health center, Nasson Health Care (NHC). NHC began back in 2004 as Spruce Street Health Center, operating two programs funded through §330 of the Public Health Services Act, targeting homeless individuals and public housing residents. In 2012, following designation of the Sanford region as a Medically Underserved Population, we expanded our scope of project to become full-fledged Community Health Center. We currently serve 3,000 patients, with the goal to expand to 6,300 by 2015. A nationally-recognized Patient-Centered Medical Home, NHC offers fully integrated primary medical, dental, behavioral health and care management services. We are York County’s first and only community health center and are one of fewer than five in the nation to be a program housed within a community action organization.
